First Visit

NEW PATIENT COORDINATOR

You will be contacted by the New Patient Coordinator prior to your first visit. His / Her job is to obtain new patient information and ensure it is correctly entered into our system and schedule your initial appointment. The New Patient Coordinator will ask that you complete our new patient medical information data sheet, the demographic and insurance form, and consent form. In addition, she will ask that you bring any x-rays, CT or MRI scans to your visit. Lastly if you have seen another orthopaedic, neurologic or pain management physician, he / she will ask that records be obtained for the physician. The New Patient Coordinator's goal is to ensure a new patient's first visit is a seamless experience with all medical information communicated to the physician so he is fully prepared to evaluate the patient.

APPOINTMENTS

Appointments are scheduled between the hours of 8:45 A.M. and 8:00 P.M. Please call as soon as you feel that you need Orthopaedic care so an appointment may be made to accommodate your schedule if at all possible. If you are unable to keep an appointment, please notify us within one business day so we may offer the appointment to another patient in need of care.

We make a sincere attempt to adhere to the appointment schedule and the patient's appointed time. However, patients do not become ill or break bones at pre-determined times, and our physicians may be delayed in surgery which may cause him to run late for his scheduled office. Your patience and understanding inthese situations isgreatly appreciated.

In order to protect your personal health and financial data, and as part of our federally-mandated identity-theft protection program, as of May 1, 2009, we are asking all of our patients to provide a photo ID or other proof of identity registration.

WHAT TO BRING

Patients requiring referrals from their primary care physiciansmust bring that referral at the time of the appointment. Lab reports, x-rays, and other diagnostic reports may also be needed at the time of service in our office. Please ask the representativewhat is needed when you schedule your appointment.

Our surgery coordinators, will provide you with all the information necessary to properly schedule your surgery, if surgery is required. Many times the location of your surgery will be dictated by your insurance provider. We will make every attempt to co-ordinate your desires with the specific surgery facility. For your convenience, our physicians are able to perform surgery at Memorial University Medical Center, Candler Hospital, and St. Joseph Hospital.
